Accident Summary
An aircraft on a flight from Teresina began its approach to Recife-Guararapes Airport runway 18. While on final approach, both engines failed simultaneously. The aircraft stalled and crashed in a residential area approximately 5 km from the runway threshold. One passenger and one pilot were killed, and eight other occupants were injured. There were no casualties on the ground. The aircraft was destroyed.
Probable Cause
The official investigation determined the probable cause to be a double engine failure caused by fuel exhaustion.
Contributing Factors
The investigation identified several contributing factors: poor flight planning, the crew's failure to add sufficient fuel before departure from Teresina Airport, insufficient fuel quantity for the required distance, failure to follow published procedures, overconfidence, poor organizational culture on the part of the operator, lack of discipline and poor judgment by the crew, and lack of supervision.
