Accident Details
A simulated engine failure was initiated by the check airman at 200 feet above ground level (AGL) during the takeoff phase. The pilot followed the prescribed emergency procedures and advanced the power levers in an attempt to restore power. After noting that the engine had failed, the pilot determined that insufficient altitude remained to perform an air restart. The subsequent descent resulted in the aircraft contacting trees located off the end of the airport runway. The descent was not arrested, and the aircraft struck the terrain, flipping inverted. All three occupants sustained injuries.
Engine Examination
Examination of the aircraft's left engine revealed foreign contamination of the fuel control governor by an air restrictor. Static testing conducted after the accident indicated significant malfunctioning at both the idle and maximum governing speed limits.

Occurrence #1: Loss of engine power (partial) - mechanical failure/malfunction. Phase of operation: takeoff - initial climb. Findings: (f) Fuel system, fuel control - disabled; (f) Fuel system, fuel control - contamination; (f) In-flight planning/decision - delayed - pilot in command; (f) Emergency procedure - simulated - pilot in command.
Occurrence #2: Forced landing. Phase of operation: descent - emergency. Findings: (c) Directional control - not maintained - pilot in command; (c) Proper climb rate - not possible - pilot in command.
Occurrence #3: In flight collision with object. Phase of operation: descent - emergency. Findings: Object - tree(s).
Occurrence #4: In flight collision with terrain/water. Phase of operation: descent - uncontrolled. Findings: Terrain condition - ground.
