A twin-engine aircraft traveling from Cleveland to Madison was on final approach to Madison-Blackhawk Airport when it experienced a sudden loss of control. During the descent, the plane nosed downward and impacted the surface of Lake Monoma. The wreckage was located approximately three miles from the runway threshold.

The accident resulted in seven fatalities among the occupants, including the musician Otis Redding. One passenger survived the impact and was successfully rescued from the site.
