Aircraft impact near Izmir-Adnan Menderes Airport

16 fatalities • Seferihisar, Turkey • Landing (descent or approach)

An aircraft crashed into a mountain slope near Izmir, Turkey, resulting in the deaths of all 16 people on board during an approach to runway 35.

What happened

During an approach to Izmir-Adnan Menderes Airport, the flight crew was executing an ILS approach to runway 35. After receiving clearance to the Outer Marker (CU NDB), the co-pilot, acting as pilot-in-command, transitioned the aircraft to the ILS frequency. During the procedure turn following the NDB, the crew failed to confirm their exact position. Consequently, the aircraft drifted outside the 35-degree sector of the ILS centerline, leading the crew to inadvertently follow the incorrect side of the beam.

As a result of this navigational error, the aircraft descended below the safe altitude and struck the slope of Mt Dümentepe, which stands at an elevation of 800 meters. The impact occurred approximately 18 km southwest of the airport. The force of the collision caused the aircraft to disintegrate upon hitting the terrain. There were 16 fatalities among the occupants of the flight.

Findings

Investigations into the crash determined that the crew's improper use of navigation aids was a primary factor in the accident. The investigation highlighted a significant failure to follow established company protocols, particularly regarding instrument flying techniques and coordination between the crew members during the approach phase. Additionally, it was noted that terminal radar services were unavailable at the time of the incident.

Probable cause

The accident resulted from the flight crew's incorrect use of navigation aids and a failure to adhere to standard operating procedures for instrument approaches and crew coordination.
