Flight Details
The crew was conducting a flight from Vancouver (Sea Island) to Holberg, with planned stops at Comox and Tofino. However, the aircraft did not stop at Comox on this trip. While flying along Vancouver Island, an engine failed.
Crash and Rescue
The pilot-in-command attempted to ditch the aircraft, which subsequently crashed into Johnstone Strait. Three crew members were killed in the accident: F/O P. C. Walker (navigator), F/O D. K. McPherson (radio operator), and AC1 N. C. Thompson. All other occupants were rescued.
Findings
The official investigation determined the probable cause to be engine failure in flight.
