What happened
During a flight traveling from Medan to Meulaboh, the aircraft experienced a significant loss of hydraulic pressure approximately thirty minutes after departure. In response to the pressure drop, the crew performed a shutdown of the left engine and feathered the propeller.
Following this action, the aircraft struggled to stay at its assigned altitude and became unstable. Consequently, the crew shut down the right engine as well. While attempting to execute an emergency landing, the aircraft impacted trees and came to rest in a swampy area. The accident resulted in one fatality and 13 injuries.
Findings
