Accident Details

The aircraft took off from runway 29 at Guaymaral-Flaminio Suárez Camacho Airport. While climbing to 200 feet, an explosion occurred in the engine, which subsequently lost power. The airplane began to descend, impacted a brick wall, and lost its undercarriage. It then crash-landed in a prairie and slid for several dozen meters before coming to rest. The accident occurred three minutes after takeoff.

All five occupants escaped the accident uninjured.

Probable Cause and Contributing Factors

The investigation determined that the accident was caused by an emergency landing on an unprepared field as a result of a decrease in power generated by engine failure. The engine failure was caused by the fracture of three blades of the rotor disk of the high-pressure compressor, which caused severe backwards damage to the hot and power section.

Contributing factors included non-compliance in the engine maintenance process with AD No. 2014-17-08R1 FAA (year 2014), which establishes the replacement of the high-pressure compressor blades in anticipation of material failures. Additionally, deficient maintenance processes by the service provider for aircraft HK4669G failed to detect the condition of the high-pressure rotor blades during boroscopic inspections. The operator also had deficient verification of the contracted maintenance processes, failing to ensure the quality and full compliance of these processes by the maintenance service provider.
