Accident Overview
During the takeoff roll at Tulsa Airport, the aircraft encountered engine problems while operating on a runway covered with slush. The pilot initiated rotation before reaching the appropriate airspeed, causing the airplane to climb only a few feet before stalling and crashing back onto the runway. The aircraft then became uncontrollable, veered off the runway, and came to rest. Both occupants suffered minor injuries, and the aircraft sustained damage beyond repair.
Probable Cause and Contributing Factors
The official investigation determined the probable cause to be an engine malfunction during the takeoff run and initial climb. Several factors were identified, including inadequate preflight preparation, premature liftoff, failure to obtain flying speed, disregard of good operating practice, and improper emergency procedures. The presence of ice and slush on the runway was also noted. Additionally, there was a partial loss of power on one engine, although engine checks prior to takeoff indicated normal operation. No full power run-up was conducted due to ice on the runway. The aircraft was rotated below VNC (velocity for minimum control), and observers reported hearing an engine backfire.
