Accident Sequence
During the takeoff roll, at the moment of rotation, the right engine failed. The aircraft then entered a skidding turn to the right. Despite the engine failure, the aircraft continued to climb, reaching an altitude of approximately 100 feet above ground level (AGL). During the climb, the aircraft turned about 110 degrees from its initial takeoff heading. While in this turn, the aircraft rolled inverted and subsequently descended, impacting a car port. Throughout this sequence, the landing gear remained extended and was not retracted. Additionally, the propellers were not feathered.
Post-Impact Fire and Damage
The collision was followed by a severe post-impact fire. The fire extensively damaged or destroyed the aircraft, the car port, and three vehicles that were present.
Examination and Casualties
A post-accident examination of the right engine revealed extensive internal damage. The accident resulted in the death of both occupants.
Probable Cause
According to the official findings, the probable cause is divided into three occurrences:
- Occurrence #1: Loss of engine power (partial) due to a mechanical failure or malfunction. The specific finding is a total failure of the engine assembly.
- Occurrence #2: Loss of control in flight, attributed to the pilot in command. Specific findings include: failure to retract the landing gear, failure to feather the propeller, failure to maintain directional control, and improper use of throttle/power control.
- Occurrence #3: In-flight collision with objects. The objects involved were identified as tree(s), a residence, and a vehicle.
