Introduction
On 11 January 2026, at 13:07 UTC, a serious incident occurred at Sazená Airport (LKSZ) involving a Zlín Z 142 aircraft, registration OK-KRP. The aircraft was operated by a private individual under non-commercial rules.
Flight Details
The pilot and a passenger had completed a 45-minute VFR flight and taxied to the fuel station with the intention of refueling. Another Zlín Z 43 aircraft was already parked at the refueling point. The crew of the Z 43 intended to taxi away from the fuel station. The pilot of the Z 142 decided to keep the engine running at idle to cool it, while the two crews agreed that the passenger would dismount and guide the Z 43 pilot during taxi to avoid contact with snow banks.
Incident Sequence
The passenger opened the canopy and stepped onto the wing walkway. Due to blown and icy snow accumulated on the walkway, the passenger slipped and fell forward off the wing. During the fall, the passenger's left arm came into contact with the rotating propeller, approximately 3 cm above the elbow. The pilot immediately shut down the engine and electrical system, exited the aircraft, and called for help.
Injuries and Response
The passenger sustained a severe injury to the left arm and was transported by helicopter to a hospital. The pilot and a nearby mechanic provided first aid, including application of a tourniquet. Police were notified and attended the scene. The incident was classified as a serious incident under EU regulation 996/2010.
Investigation Findings
The investigation, conducted by the Czech Accident Investigation Institute (ÚZPLN), found no technical defects with the aircraft. The pilot was qualified for the flight. Meteorological conditions included snow and ice on the wing surface.
Cause
The probable cause of the serious incident was the fall of the passenger from the aircraft wing and subsequent collision of his left hand with the rotating propeller.
