History of the Flight
On 24 June 2014, at approximately 11:20 UTC, a Robinson R22 Beta 2 helicopter, registration 2 VE-HJB, departed from Farm Immenhof in the Omaruru area, Erongo Region, Namibia. The purpose of the flight was to capture kudus (Tragelaphus strepsiceros) for transport to Mooiplaas Farm for breeding. The pilot was the sole occupant. The mission involved herding kudus from Farm Immenhof.
During the operation, after two successful captures, the pilot observed two kudus breaking away from the herd. While maneuvering to herd them back, the pilot flew from west to east and inadvertently flew into a single-phase electrical cable that ran north-south. The landing skids hooked the cable, causing the helicopter to flip over and lose control, crashing onto its left side.
Injuries and Damage
The pilot sustained minor injuries and was trapped in the wreckage until farm workers lifted the helicopter upright. The helicopter sustained damage to its rotor system and airframe. No other injuries were reported.
Pilot and Aircraft Information
The pilot held a valid Commercial Pilot License (Helicopter) with no restrictions, and had accumulated approximately 4,000 total flying hours, including 3,300 hours on type. The aircraft, a Robinson R22 Beta 2, had a valid Certificate of Airworthiness. The last Mandatory Periodic Inspection (MPI) was certified on 23 June 2014 at 2,913.5 airframe hours by an approved South African AMO (NAMCAR Part 145 approval). At the time of the accident, the aircraft had accumulated an additional 10 hours since the MPI.
Meteorological Information
Weather conditions were fine with good visibility. Wind was northerly at 6.4 knots, temperature 25°C, CAVOK. Dew point was not recorded.
Investigation
The accident was reported by a regulatory inspector present at the farm. The Directorate of Aircraft Accident Investigation (DAAI) conducted the investigation. No foreign authorities were notified, and no accredited representatives were required.
Probable Cause
The investigation determined the probable cause as: "Aircraft struck a single electrical cable." A contributing factor was "Loss of situational awareness which caused the aircraft to be flown into a single electrical cable."
