What happened
On the evening of an unspecified date in August 2013, a float-equipped DHC-3 turbine Otter, registered as C-FSGD, began a scheduled daytime flight under visual flight rules. Operated by Transwest Air Limited Partnership, the aircraft departed from Scott Lake in the Northwest Territories, intending to travel 33 nautical miles to Ivanhoe Lake.
Following the departure, the aircraft failed to reach its intended destination. By approximately 2100 Central Standard Time, the operator reported the aircraft as overdue. Despite the notification to the Joint Rescue Coordination Centre Trenton, no signals were detected from the aircraft's emergency locator transmitter.
A search operation involving a C-130 Hercules was initiated to locate the missing plane. On 23 August 2013, search crews discovered the wreckage situated in an unnamed lake, located roughly 10 nautical miles north of the aircraft's last known position. The pilot, who was the only person on board, suffered fatal injuries in the crash.
