Accident Details
On 28 April 2023, at approximately 14:10 local time (20:10 UTC), a Leonardo Agusta Westland AW-109E helicopter, registration TI-BJX, operated by Helicópteros del Norte S.A., was involved in an accident. The aircraft was on a commercial flight from Tobías Bolaños International Airport (MRPV) to Roca Bruja, Playa Naranjo, Guanacaste. The accident occurred about 3.94 nautical miles south of Daniel Oduber Quirós International Airport (MRLB), in a field owned by Central Azucarera del Tempisque S.A. (CATSA) in Liberia, Guanacaste, Costa Rica.
While en route, approximately 3 nautical miles south of MRLB and nearing the intended landing point, the helicopter experienced an abrupt left yaw and began an uncontrolled descent, impacting the ground. The flight was conducted in daytime visual flight rules (VFR) conditions.
Injuries and Damage
All six occupants—one pilot and five passengers—survived the accident. According to the investigation authority, four occupants were uninjured, one sustained serious injuries (the pilot), and one sustained minor injuries. Both injured individuals were transported to a medical center. The helicopter was destroyed, suffering major structural damage. There was no fire before or after the impact.
Investigation Status
The investigation is being conducted by the Unidad de Accidentes e Incidentes Aéreos (UAI-CETAC) of Costa Rica. The focus has been on photographic and video evidence, as well as the wreckage. The two Pratt & Whitney PW206C engines and other components are to be sent to their respective manufacturers for detailed analysis. The investigation remains ongoing, and further actions are pending staffing decisions within CETAC. A final report is expected after component examinations and international coordination with agencies including the NTSB, TSB, and Italy's aviation safety agency.
