What happened
On July 31, 1999, at 17:07 UTC, a Cessna 172 (R172H) crashed into terrain near the Uyuimi airstrip in Ecuador. The aircraft was operating as an unscheduled passenger flight, transporting two adults and one minor from Shell Mera's Río Amazonas airport.
The pilot had completed two successful flights earlier that day under favorable conditions. After filing a VFR flight plan to fly at 5,500 feet, the aircraft departed at 16:53 UTC. The last communication with air traffic control indicated an estimated overflight of Pitirishca at 17:07 UTC.
During the approach, the aircraft struck two 30-meter trees atop a hill at an elevation of 3,300 feet. This initial impact shattered the windshield and sent debris forward. The aircraft then struck the ground 90 meters further along the flight path, where it caught fire due to the contact of fuel and hydraulic fluid with hot engine components. All four fatalities (the pilot and three passengers) were caused by the trauma of the impact.
The investigation
The investigation focused on the meteorological conditions, the mechanical state of the aircraft, and the pilot's decision-making. Investigators examined the wreckage, including the engine, propeller, and control surfaces.
Technical analysis of the Continental IO-360 engine and the McCauley propeller revealed that both were functioning normally at the time of impact. While some minor maintenance irregularities were noted on the propeller hub—such as the improper use of sealant which could mask hydraulic leaks—these were not determined to be contributing factors. The fuel quality was also confirmed to meet all required specifications.
Meteorological evidence from local residents and other pilots confirmed that a sudden, dense fog had descended upon the area, reducing visibility to approximately 10 meters. This weather pattern was noted as being frequent for that time of year.
