Flight Details and Accident Overview On July 31, 1999, a Cessna 172 (model R172H, serial R-1720546) was conducting a non-scheduled passenger transport flight from Shell Mera's Río Amazonas airport to Uyuimi. The aircraft departed at 16:53 UTC under a Visual Flight Rules (VFR) plan, carrying one pilot and three passengers. The pilot's final contact with the control tower was at 17:07 UTC, when he estimated he would overfly Pitirishca. The aircraft was subsequently found destroyed by fire on the side of a hill at 3,300 feet elevation, 10 nautical miles from Uyuimi runway. ## Casualties and Damage All four occupants on board—one pilot and three passengers—were killed in the accident. The pilot and passengers died from traumatic injuries sustained during the impact. The aircraft was completely destroyed by the impact and subsequent fire, which was caused by fuel and hydraulic fluid contacting hot engine parts. No surface damage was reported. ## Crew and Aircraft Information The pilot held a valid Commercial Pilot License with no medical restrictions. At the time of the accident, he had logged 1,538:59 total flight hours, including 413:02 hours in the Cessna 172. He was contracted for the day's flights because the company's regular pilot had reached his monthly hour limit. The aircraft maintained a valid airworthiness certificate and was up to date with maintenance, including a 1,000-hour inspection completed on July 17, 1999. ## Meteorological and Impact Conditions Witnesses reported dense fog in the area at the time of the accident, limiting visibility to less than 10 meters. The weather deteriorated rapidly and cleared shortly after the crash. The initial impact occurred with two 30-meter trees on a hilltop, shattering the windshield. The final impact happened 90 meters later on another small hill, where the engine and propeller were buried in tree roots. ## Investigation Findings Examination of the wreckage ruled out in-flight separation, as all major components were found at the final impact site. Control cables showed fire damage but were not broken. The engine was found to be in good mechanical condition prior to the accident, with no evidence of malfunction. The propeller was rotating at the time of the first impact, with its blades bent backward 90 degrees. Although minor discrepancies were noted in the propeller installation, investigators determined these were not contributing factors to the accident. The fuel samples met purity and quality specifications. The investigation concluded that the dense fog prevented the pilot from maintaining visual contact with the terrain, making VFR flight conditions unsuitable.
