What happened
The incident occurred during a local test flight departing from Offutt Air Force Base. The purpose of the sortie was to verify performance after both engines had been replaced. Immediately following takeoff, the aircraft failed to climb and lost altitude. Unable to maintain flight, the airplane drifted southeastward over the terrain.
The aircraft struck the upper branches of bottom willow trees before reaching the edge of a nearby river. Upon impact with the ground, the nose struck first, causing the airframe to flip onto its back. The structure subsequently caught fire. Both members of the crew were quickly extracted from the wreckage and sustained injuries.
Findings
The official investigation attributed the accident to engine failure. This mechanical issue prevented the aircraft from gaining necessary altitude after departure.
