What happened
American Airlines Flight AA331, a Boeing 737-823 registered N977AN, departed Miami (KMIA) on December 22, 2009, bound for Norman Manley International Airport (MKJP) in Kingston, Jamaica. The aircraft was carrying 148 passengers and a crew of six under instrument flight rules.
During the approach to runway 12 at night, the crew utilized an Instrument Landing System (ILS) approach with a heads-up display (HUD), becoming visual approximately two miles from the runway threshold. The aircraft landed in heavy rain with a 14-knot left quartering tailwind. Touchdown occurred at approximately 4,100 feet on the 8,911-foot runway.
Despite the crew's efforts to decelerate, the aircraft was unable to stop within the remaining 4,811 feet of usable runway. It overran the end of the runway at a ground speed of 62 knots. The aircraft broke through a perimeter fence, crossed above a road situated below the runway level, and came to a halt on the sand dunes and rocks between the road and the Caribbean Sea.
The impact resulted in the destruction of the aircraft, with the fuselage breaking into three sections. The left landing gear collapsed, and the right engine and landing gear were torn off. The left wingtip sustained significant damage, and the right wing fuel tanks ruptured, leaking jet fuel onto the beach. There was no post-crash fire.
Regarding injuries, 134 passengers suffered minor or no injuries. Fourteen passengers sustained serious injuries, though none were life-threatening. All flight crew and cabin crew members escaped without serious injury and assisted in the evacuation of the aircraft.
The investigation
The Jamaica Civil Aviation Authority (JCAA) investigated the occurrence in accordance with Annex 13 to the Convention on International Civil Aviation. The National Transportation Safety Board (NTSB) of the United States served as the Accredited Representative. The investigation focused on the factual circumstances of the flight, aircraft performance, weather conditions, and airport infrastructure.
The inquiry examined the runway surface friction measurements, which indicated reduced braking effectiveness due to heavy rainfall. It also reviewed the crew's decision-making process during the approach and landing, including the use of reverse thrust and braking systems. The structural integrity of the aircraft following the overrun was analyzed to understand the extent of the damage.
Findings
The primary causal factors for this accident were the runway overrun due to reduced braking action on a wet runway and the insufficient runway end safety area at Norman Manley International Airport. Contributing factors included the heavy rain conditions, the presence of a tailwind during landing, and the lack of a paved runway end safety area or blast pad at the threshold of runway 12.
The investigation established that the aircraft landed with sufficient remaining runway distance to stop under dry conditions, but the wet surface significantly increased the required stopping distance. The absence of a designated safe area beyond the runway end allowed the aircraft to continue its momentum into hazardous terrain.
Safety action
The JCAA issued several safety recommendations following the investigation. Key actions included:
- Implementing recurrent training and proficiency checks for air traffic controllers.
- Investigating modifications to Boeing 737 overhead bins and passenger service units to prevent passenger injuries in similar incidents.
- Reviewing requirements for captain’s seat belt crotch brackets to prevent failure during high-load events.
- Prohibiting the use of uncertified replacement seat cushions that compromise occupant protection.
- Recommending runway grooving at NMIA to improve tire traction and drainage during adverse weather.
- Coordinating a full assessment of runway pavement, slopes, and friction levels.
- Enhancing aerodrome emergency plans, particularly regarding water rescue capabilities.
- Ensuring consistency in FAA advisory material regarding wet runway definitions.
- Requiring the recording of ATIS broadcasts for future investigation purposes.
- Aligning JCAA Air Traffic Services procedures with ICAO provisions.
