What happened
On August 8, 2022, at approximately 17:00 UTC, a Cessna 421, registration YV1207, was conducting a functional check flight at Metropolitano Airport (SVMP) in Ocumare del Tuy, Venezuela. The flight, operated by Inversiones 421-0244 C.A., was intended to be a local circuit for testing purposes.
After completing engine start and initial brake checks, the crew taxied the aircraft to the general ramp for security inspections. Following these checks, the pilot requested authorization to taxi to the holding point for a run-up. As the aircraft approached the holding point for runway 08, near the windsock, the pilot attempted to decelerate but found that the right brake failed to respond. Despite repeated attempts by both the pilot and the co-pilot to apply braking, the aircraft continued moving, veering to the left. The Air Traffic Controller lost visual contact with the aircraft as it exited the taxiway and entered the safety area adjacent to the runway. There were no injuries to the crew and no damage to the aircraft.
The investigation
JIAAC investigators conducted an on-site inspection of the aircraft, documenting its position between the runway and the grass area. The investigation included a physical examination of the braking system for leaks or cracks, as well as a functional taxi test to verify brake performance. Investigators also reviewed maintenance records from the responsible maintenance organization, OMAC-N258 AVIOCENTER SERVICIOS AÉREOS, C.A., and verified the pilot's credentials and medical certification. Preliminary inspections of the hydraulic lines showed no signs of fluid leakage.
