Incident Overview and Aircraft Details On August 21, 2022, at 16:29 UTC, a McDonnell Douglas MD-83 (DC-9-83) with registration YV3465 and serial number 49452, operated by Linea Aerea de Servicio Ejecutivo Regional (LASER) C.A., began a takeoff roll from Maiquetía-Simón Bolívar International Airport (SVMI) in Venezuela. The flight was scheduled to Maturín (SVMT) with 101 people on board, including two pilots, three flight attendants, one other crew member, and 92 passengers. The aircraft was powered by two Pratt & Whitney JT8D-219 turbofan engines and was certified for transport category operations. At the time of the incident, the aircraft held valid airworthiness, registration, and radio station certificates issued by the Venezuelan aeronautical authority, INAC, and was covered by valid insurance. The maintenance organization supporting the operator was OMAC N°475 LASER AIRLINES, C.A., located at the same airport. ## Operational Sequence and Investigation Findings During the takeoff roll, the flight crew observed erroneous indications for N1, N2, and EGT on the right engine. The crew aborted the takeoff, vacated the runway without incident, and taxi to the apron to deplane the passengers. Weather conditions were favorable, with zero-knot winds, unlimited visibility, and generally good conditions throughout the day. The investigation, conducted by the Venezuelan civil aviation accident investigation board (JIAAC), included reviewing maintenance records, crew documentation, and interviewing the flight crew. The first officer, aged 40, held valid licenses and ratings for instrument flight, single-engine land (C172), and MD80 First Officer/SIC. During inspection of the right engine, investigators detected a hydraulic leak in the PT7 hydraulic line. The maintenance organization replaced the PT7 hydraulic line in accordance with AMM 20-12-01, page 205, under the supervision of INAC airworthiness inspectors and the lead investigator. A subsequent test run of the right engine system was performed and found to be within parameters per AMM 71-00-00, page 506. The investigation was ongoing at the time of the preliminary report, with technical, operational, and documentary analysis in progress. The incident was reported to the ICAO ADREP system, and the United States, as the state of design and manufacture, was notified. The NTSB responded by assigning an accredited representative.
