What happened

On November 16, 2021, at 15:35 local time, an EMB-145LR, registration JU-1800, operated by Aeromongolia, was performing a scheduled flight from Chinggis Khaan International Airport in Ulaanbaatar to Donoy. During the take-off roll on runway 29, specifically at 1,560 meters from the threshold, the #2 main wheel tire burst. The aircraft continued its rotation, lifting off at approximately 1,960 meters at a speed of 158 knots.

Upon arrival at Donoy, ground technicians reported hearing unusual noises from the aircraft exterior. A subsequent inspection revealed that the #2 main wheel tire had burst, with approximately 30% of the tread stripped away. The incident also caused secondary damage to the aircraft, including damage to the inboard flap and wiring for the brake temperature and proximity sensors, as well as damage to the front section of the left main gear.

The investigation

The AAIB investigation examined the aircraft's maintenance history, tire pressure logs, and operational records. Investigators reviewed the tire's history, noting the component had been installed on October 7, 2021. The investigation also scrutinized the airline's maintenance program, which required tire pressure checks every 48 hours, and compared this against manufacturer recommendations for 24-hour checks.

Furthermore, the investigators analyzed the aircraft's recent operational performance, identifying several instances where flight limitations were exceeded during taxi and landing phases. The investigation also looked into the aerodrome environment, discovering a small piece of metal (approximately 5cm) near the parking stand, though its direct link to the tire burst was not established.
