What happened

On 9 June 2020, an AT-802A aircraft, registration PK-PAG, was performing an unscheduled fuel hauling mission for Pelita Air Service. After completing an initial successful flight leg from Timika to Karubaga, the aircraft returned to Timika to reload 3,500 liters of diesel fuel.

During the second flight leg back to Karubaga, the pilot conducted several brake checks during taxi and the downwind approach, noting a slight difference in pedal pressure between the left and right brakes but ultimately deeming them functional. Upon approaching runway 17 at Karubaga, the aircraft performed a three-point landing. Following the initial touchdown, the aircraft bounced slightly to the right of the runway centerline and landed again approximately 30 meters further down the strip. In an attempt to correct the aircraft's position and return to the centerline, the pilot applied the left brake. This maneuver caused the aircraft to veer sharply to the left, exiting the runway and striking the airport perimeter fence.

The investigation

The KNKT investigation examined the aircraft's mechanical condition, the pilot's recent flight history, and the operational procedures for fuel hauling. The investigation noted that the aircraft was wet-leased from a Canadian operator, which was responsible for maintenance and crew training.

Investigators analyzed the impact site, finding braking marks from both wheels starting 200 meters from the runway threshold. The aircraft's left wing tip, left wing leading edge, and propeller blades sustained damage during the collision with the fence. The investigation also attempted to retrieve data from the Data Acquisition Alarm Monitor (DAAM), but the memory card was found to be corrupted.
