What happened
On 9 May 2023, a Cessna C208, registered PK-HVG, was performing an unscheduled cargo flight from Dekai to Yabi Airstrip in Papua, Indonesia. The flight, operated by Dimonim Air, was conducted under Visual Flight Rules (VFR) with two pilots on board. After a successful flyover to assess the runway condition, the aircraft commenced its approach and touched down approximately 100 meters from the threshold.
As the aircraft rolled along the upslope runway, the nose landing gear collapsed and folded backward. The aircraft then veered toward the right side of the runway. In an attempt to maintain control, the pilot applied full braking and left rudder, but the aircraft continued to move uncontrollably off the runway surface. The aircraft eventually struck a large rock, which caused the propeller to detach from the engine power drive. The aircraft came to a stop approximately 330 meters from the runway threshold. There were no injuries to the crew, though the aircraft sustained substantial damage, including a collapsed nose gear, bent propeller blades, and damage to the engine cowling and exhaust.
The investigation
The KNKT examined the wreckage, flight data from the aircraft's GPS, and the operational documentation provided by the operator. Investigators identified a pothole in the nose wheel path on the soggy ground near the impact site. The investigation also reviewed the pilot's aeronautical charts and the company's internal operational manuals to determine if sufficient information was available for the flight.
Findings
- The aircraft's nose landing gear failed, leading to a loss of directional control.
- The aircraft's internal operational manual (OM-C) lacked specific information regarding the Yabi airstrip and did not provide guidance for the Dekai to Yabi route.
- Inaccurate aeronautical data was present in the pilot's possession; the pilot's chart listed a runway designation of 10/28 and a touchdown elevation of 6,800 feet, whereas the actual runway heading was 08/26 and the elevation was 7,300 feet.
Safety action
To address the identified discrepancies, the KNKT issued a safety recommendation to Dimonim Air. The operator is advised to ensure that pilots are provided with accurate and comprehensive references, specifically including updated information for the Yabi airstrip and detailed guidance for operations between Dekai and Yabi.
