On 11 August 2018, a PAC 750XL (PK-HVQ) operated by Dimonim Air crashed near Oksibil after departing Tanah Merah in weather below VFR minima. Eight occupants were killed and one seriously injured.

Flight details and departure context On 11 August 2018, a PAC 750XL aircraft registered PK-HVQ was operated by PT. Marta Buana Abadi (Dimonim Air) on an unscheduled passenger flight from Tanah Merah to Oksibil. At the time of the occurrence, meteorological conditions at Oksibil were below Visual Flight Rule (VFR) weather minima and did not improve. The pilot nonetheless decided to proceed to Oksibil after learning that other flights had completed the route and returned. At 1342 LT, in daylight, the aircraft departed Tanah Merah with one pilot, one observer pilot, and seven passengers aboard. The passenger and cargo manifest listed a total passenger and baggage weight of 473 kg. No record or report of an aircraft system malfunction existed before departure. ## Approach and loss of contact At 1411 LT, the pilot made initial contact with Oksibil Tower and reported maintaining an altitude of 7,000 feet, with an estimated arrival time of 0520 UTC (1420 LT). The controller advised that visibility was 1 to 2 km and that most of the area was covered by cloud. At 1416 LT, the pilot reported being over Oksibil Airport. The controller instructed the pilot to continue to the final for runway 11 and to report when the runway was in sight. Subsequent calls from the Oksibil Tower controller and pilots of other aircraft received no reply. ## Wreckage location and casualties The following day, the aircraft was found on a mountain ridge approximately 3.8 nautical miles northwest of Oksibil, on a bearing of 331°, at an elevation of about 6,800 feet. Eight occupants were fatally injured, and one occupant was seriously injured.

Probable cause

The KNKT concluded that the VFR weather minimum requirement was not properly implemented, most likely leaving the pilot without a clear visual of the surrounding area. The Pilot in Command also lacked knowledge of the terrain around Oksibil, and the absence of a TAWS voice alert while flying close to terrain meant the PIC did not have adequate terrain awareness while flying into clouds, continuing below terrain height until impact.
