Incident Overview and Flight History On 19 May 2024, a Tecnam P2006T aircraft with registration PK-IFP was operated by Perkumpulan Penerbangan Indonesia (PPI) for a joy flight. The scheduled route was from Pondok Cabe Airport (WIHP) in South Tangerang to Salakanagara Airport Tanjung Lesung (WIHI) in Pandeglang and back. The aircraft carried one pilot and two passengers, including the aircraft engineer and a PPI pilot member. The flight was conducted under Visual Flight Rules (VFR), with a filed flight plan indicating sufficient fuel endurance for a four-hour flight. The aircraft took off from Pondok Cabe using Runway 36 at 1141 LT, climbing to a cruising altitude of 1,500 feet. During the takeoff roll and the approach to Tanjung Lesung, the aircraft engineer recorded cockpit videos from the backseat. These videos showed that the left tank fuel quantity indicator needle was below the 0 lt value (on the electrical zero), while the right tank indicator was between 30 lt and 50 lt. At 1222 LT, the aircraft landed at Tanjung Lesung using Runway 25 and parked at the beginning of Runway 07. All occupants disembarked for a rest. Around 1230 LT, they embarked for the return flight and took off using Runway 07. During this takeoff roll, the engineer recorded another video showing the left fuel quantity indicator still below the 0 lt value. At 1310 LT, the aircraft departed Tanjung Lesung and climbed to 1,500 feet. At 1341 LT, ATC surveillance recorded the aircraft abeam Waypoint GOLFI at 1,000 feet, continuing to descend. At 1342 LT, the pilot transmitted a distress message to Halim Tower: “MAYDAY, MAYDAY, MAYDAY. INDIA FOXTROT PAPA a…make fo...”. The message was acknowledged, and the aircraft was recorded at 600 feet. The aircraft crashed near an open field approximately 1.5 NM on bearing 146° from Waypoint GOLFI.
