What happened
During the final days of the Republic of Vietnam, an aircraft operating as part of 'Operation Babylift' departed Saigon-Tan Son Nhat Airport at 1603LT. The mission was intended to transport Vietnamese orphans to the United States. Approximately twelve minutes after takeoff, while cruising at 23,000 feet over the South China Sea, a mechanical failure involving the rear access ramp doors led to a rapid decompression of the cabin. During this event, one crew member fell from the aircraft, and the access ramp door subsequently detached and fell into the ocean.
The flight crew declared an emergency and received clearance to return to Saigon. However, the aircraft suffered severe damage; two of its four hydraulic systems were non-functional, and the control cables for the elevators and rudder had been severed. This left only one aileron and the wing spoilers operational. During the approach to runway 25L, the aircraft's descent rate accelerated to 4,000 feet per minute. The Boeing 727 struck the ground approximately 4 km before the runway at 250 knots, bounced, and finally crashed into a rice paddy field 500 meters further along the path. The impact caused the aircraft to disintegrate across a large area.
The flight was carrying 149 orphans, 16 crew members, 155 military personnel, and 10 medical staff. The accident resulted in 141 orphans, six soldiers, five crew members, and three medical team members killed.
Findings
Investigations into the recovered rear doors revealed that several locks had failed to engage properly. Maintenance history indicated that components had been cannibalized for parts and then incorrectly reinstalled. Although an initial inspection by one maintenance shift identified that five of the seven locks were non-functional—which should have grounded the aircraft—the flight was cleared following a second inspection by an off-shift team that failed to notice the defect. This oversight was reportedly influenced by organizational pressure to depart. The crew also noted difficulty securing the doors prior to takeoff. As the aircraft reached higher altitudes, the increasing pressure differential caused the remaining functional locks to fail.
