On September 16, 2021, the pilot of the White Swan aircraft, registration RA-1815G, intended to conduct two flights within uncontrolled class G airspace. The first flight was planned to a recreation area near Lake Shumpavlenskoye, and the second flight was intended to return to the aircraft's base at Lake Glubokoye. The pilot did not notify air traffic control authorities of these flight plans, which is a violation of Russian Federal Aviation Regulations.
According to witness statements, the pilot and a passenger arrived at the aircraft's base at approximately 07:00. The pilot managed the pre-flight preparations, including refueling and loading property. A security guard assisted the pilot with taxiing and launching the seaplane into the water from its hangar on the shore of Lake Glubokoye.
Because the aircraft lacked objective onboard control equipment, flight history details were reconstructed using engine control module data and witness accounts. The accident resulted in two fatalities.
