On November 8, 2022, a Mi-2 helicopter, registration RA-14185, was engaged in an urgent medical mission to provide assistance to a critically ill patient. The flight was scheduled to travel from Kostroma (Sokerkino) airfield to the village of Sharya and return to the departure airfield. The mission was conducted under the authority of a contract with the Kostroma Regional Clinical Hospital.
Prior to departure, the captain underwent a medical examination and noted a discrepancy between his minimum requirements and the predicted weather conditions. Despite this, the flight plan was submitted and IFR permission for airspace classes C and G was obtained. The crew consisted of a captain and a first officer.
The accident occurred at night during the flight phase. The aircraft RA-14185 collided with the ground approximately 2 km northeast of the Kostroma (Sokerkino) airfield. The impact resulted in one fatality and no other injuries.
Investigations revealed that the pilot-in-command made the decision to operate the aircraft in weather conditions that did not meet regulatory standards for night flight. The crew failed to transition to instrument flight as visibility decreased, leading to a loss of spatial orientation.
