Flight Details
On June 21, 2016, a pilot operating a helicopter Mi-2 registered RA-23709 performed two medical transport flights. The first flight followed the route Reshety – Yugush – OKB No 1, after which the helicopter returned to Reshety. The second flight went from Reshety to Bisert and then to OKB No 1. At 14:00, the pilot took off from Bisert to transport a patient to the Sverdlovsk Regional Clinical Hospital No 1 (OKB No 1) in Yekaterinburg. On board were the pilot, the patient, and two medical workers. The flight to OKB No 1 was conducted at an altitude of 200 meters.
Accident Sequence
After reaching the check point at 14:34, the pilot inspected the landing pad and initiated an approach with a magnetic course of 290°. At 14:37:45, the pilot performed a vertical landing on the pad at OKB No 1. Following landing and engine shutdown, the patient and accompanying medical workers disembarked. The pilot planned to then fly the helicopter back to its base at Reshety. At 14:50, the pilot lifted off with a magnetic course of 290°. After executing a right turn to a magnetic course of 110°, the pilot attempted a vertical takeoff with acceleration while in ground effect. During the takeoff, a significant increase in pitch angle occurred, and the tail rotor made contact with the ground. Subsequently, the helicopter turned left, struck the ground, and rolled onto its right side. At the time of the accident, the helicopter carried only the pilot and a 50 kg service load consisting of a helicopter medical kit.
Aftermath
The pilot reported the accident to the management of the airline company OOO "APK VECTOR". Further notifications were handled by the company.
