What happened
On August 15, 2015, a Mil Mi-8T, registration RA-22559, was involved in an aviation accident in the Tuguro-Chumikansky District of the Khabarovsk Krai. The aircraft was operated by JSC Vostok Airlines. The flight was a scheduled commercial operation intended to transport 13 passengers along a route connecting Khabarovsk, the P. Osipenko settlement, and Ongachan Bay.
The flight crew, consisting of a pilot-in-command, a co-pilot, and a flight engineer, had completed pre-flight preparations at Khabarovsk (VNL) airfield. The crew had received meteorological information regarding current and forecasted conditions for the departure airport, the intermediate landing site at P. Osipenko, and the alternate airport at Khabarovsk (Novy) in the form of GAMET forecasts. The flight was conducted under a charter agreement established between a private individual and the operator.
The investigation
The investigation reviewed the flight planning and crew preparation processes. Records indicate that the flight crew underwent seasonal training and preparation with the management of Vostok Airlines on March 23, 2015. The flight request for the specific route was submitted to the Khabarovsk Air Traffic Control Center on August 14, 2015. Pre-flight medical examinations for the crew were conducted at Khabarovsk (VNL) on August 14, 2015, between 21:00 and 21:30 UTC.
