An aircraft flying from Biak to the Netherlands crashed near Abadan, Iran, resulting in the deaths of all ten people on board.

What happened

During a ferry flight from Biak to the Netherlands, a seaplane was forced to divert to Abadan due to mechanical issues. Following a period of two weeks for repairs, the crew attempted to resume the journey toward Europe. However, shortly after departing from Abadan Airport, an oil leak was detected in the first engine, necessitating an immediate return to the airfield.

As the aircraft approached the airport, it became uncontrollable and impacted the ground near the runway. The impact resulted in a massive explosion. There were ten fatalities among the crew members on board.

Findings

Investigations into the accident focused on the malfunction of the engine's systems. It was determined that the thrust reversal system on engine number one was activated unintentionally, causing the aircraft to lose control during the approach.

Probable cause

The accidental deployment of the thrust reversal system on the first engine caused the aircraft to become uncontrollable during its approach to the airport.
