Accident
At 03:43 UTC during nighttime, an Antonov An-12BK, registration UR-CAH, crashed while on approach to Lviv airport in conditions of limited visibility due to fog. The aircraft was operated by PrJSC "Aviation Company "Ukraine-Aeroalliance" on flight UKL4050 from Vigo, Spain, to Lviv, Ukraine.
Casualties and Damage
The accident resulted in the deaths of five flight crew members. Two aviation technicians and a relief flight engineer who were also on board sustained serious bodily injuries. The aircraft was destroyed and deemed irreparable.
Investigation
The investigation was completed on 1 October 2021. The National Bureau of Air Accidents Investigation of Ukraine published the final report.
