Flight History
On 16 February 2021, a Boeing 737-500 (registration 5N-SYS) operated by Azman Air Services Limited as flight AZM2325 departed Nnamdi Azikiwe International Airport, Abuja, for Murtala Muhammed International Airport, Lagos. While on approach to runway 18L, the flight crew executed a missed approach due to adverse weather. Approximately 12 minutes later, the crew reported being established on the ILS for runway 18R. The aircraft was cleared to land. After touchdown, the aircraft passed the Alpha 2 intersection on the runway, and all main landing wheel tyres burst. The aircraft sustained substantial damage.
Aircraft and System Details
The aircraft was a Boeing 737-500, serial number 28565, manufactured in 1997. At the time of the incident, the anti-skid system was unserviceable. The flight was conducted in accordance with the Minimum Equipment List (MEL) that allowed operation with the anti-skid inoperative; however, the investigation noted that MEL extension requirements were not fully satisfied. The runway surface was wet.
Investigation Findings
The Nigerian Safety Investigation Bureau (NSIB) determined the causal factor to be the locking of the main landing gear wheels during the landing roll on a wet runway due to the unserviceable anti-skid system. Contributory factors included the decision to operate the flight with the anti-skid inoperative and the extension of MEL requirements.
Damage and Recovery
The burst tyres generated debris that caused damage to the number 1 engine fan blades and the flap track fairing. No fire occurred. After tyre replacement, the aircraft was towed to a military hangar at DNMM. The emergency response included the Lagos State Emergency Management Agency and other agencies.
