Aircraft impact following unsuccessful ILS approach attempts

1 fatality • San Antonio, United States of America • Landing (descent or approach)

An aircraft disappeared from radar after the pilot struggled to maintain a coupled approach, eventually impacting a field and a barn.

What happened

During the approach phase, the pilot made three separate attempts to intercept an instrument landing system (ILS) localizer. During these maneuvers, the pilot communicated with Air Traffic Approach Control, reporting difficulties maintaining a coupled approach and expressing a need to regain control of the aircraft. Following these communications, the aircraft vanished from radar surveillance. The flight path ended when the aircraft struck a field before colliding with a barn. The impact left the aircraft in an upright position, which led to the development of a post-crash fire.

Findings

Investigation of voice and radar records indicated that the pilot was capable of executing heading changes and altitude adjustments as directed by air traffic control during the three approach attempts. Mechanical inspections of the engine and airframe revealed no malfunctions or anomalies that would have hindered the normal operation of the aircraft. However, toxicological analysis of the pilot following the accident showed the presence of a regular prescription antidepressant. Additionally, the results indicated the recent ingestion of an over-the-counter antihistamine at a dose exceeding the recommended maximum, a substance known to cause impairment.

Probable cause

The pilot's ability to control the aircraft was likely impaired by the recent use of an excessive dose of an over-the-counter antihistamine.
