What happened

On 9 April 2014, a Kodiak-100 aircraft, registered PK-SDF, was performing a non-scheduled flight operated by PT. Adventist Aviation Indonesia. The flight was traveling from Doyo Baru Airstrip toward Ninia Airstrip in Papua. The aircraft was carrying a total of seven people, including one pilot and six passengers. Prior to this leg, the pilot had completed three other flight segments that day between Doyo Baru, Puldamat, and Soya.

At approximately 0015 UTC, the pilot contacted the Sentani Tower controller to request engine start and clearance for the flight to Ninia. After receiving approval, the pilot notified the controller that the aircraft was ready for departure at 0021 UTC. The controller instructed the aircraft to hold briefly to allow another plane to depart from Sentani Airport. Although takeoff clearance was granted at 0024 UTC, the pilot failed to report being airborne as instructed.

By 0027 UTC, the controller attempted to contact the aircraft, but there was no response. After failing to visually locate the aircraft in the Doyo Baru area, the controller notified the Chief Section of Sentani Tower Air Navigation. Information subsequently obtained from another local pilot indicated that an accident had occurred during the takeoff phase at Doyo Baru. Upon arriving at the site near a river, responders observed smoke emanating from the wreckage. Local residents and staff were observed attempting to suppress an engine fire using water and extinguishers while assisting survivors.

Findings

The accident resulted in two fatalities and five serious injuries. All individuals on board were transported to Yowari Hospital following the crash.
