What happened
During a departure from Ione Airfield, a twin engine aircraft experienced mechanical complications shortly after beginning its takeoff roll. As the flight progressed down the runway, the pilot did not initiate an aborted takeoff despite the emerging difficulties. The rotation of the aircraft occurred significantly later than required for a safe climb. Consequently, the plane collided with a dirt embankment and subsequently crashed at the far end of the runway. The impact resulted in the destruction of the aircraft, the death of the pilot, and injuries to three passengers.
Findings
Investigations into the accident identified that the primary factor was inadequate flight preparation by the pilot. Specifically, the crew failed to adhere to the standard pre-departure checklist, which led to a failure to notice that the gust locks remained engaged during the takeoff sequence.
