On October 28, 2014, a Robinson Helicopter Company Robinson R44 II (registration 9XR-SJ) operated by Akagera Aviation was involved in an aviation accident near Kabuguzo Village, Kintarure Cell, Shyira Sector, Nyabihu District, Rwanda, RW. Investigators recorded the probable cause as: The pilot's failure to execute a proper flare during a precautionary landing in a nose-heavy helicopter, compounded by high density altitude, downdrafts, gross weight near limitations, and low-level flight in complex terrain. This summary draws on records from the Rwanda Aircraft Accident Investigation Department (AAID); 1 related events involving the same aircraft type or operator are linked below.

On October 28, 2014, a Robinson R44 II performing a charter flight in Rwanda made a precautionary landing due to low RPM from downdrafts. The aircraft sustained substantial damage, but all four occupants evacuated without injury.

Accident

On October 28, 2014, a Robinson R44 II helicopter operated by Akagera Aviation was engaged in an unlanded charter flight between Gisenyi and Kigali airports in Rwanda.

Flight Details

The aircraft departed with a gross weight of 2,473 lbs, near its maximum certified limit. Its center of gravity was positioned forward of the optimal range. During the return leg, the pilot maintained a low-altitude profile through high-terrain valleys to accommodate a passenger suffering from severe aerophobia.

Incident Description

While navigating a narrow pass, the helicopter encountered strong downdrafts and mountain winds. These conditions caused a significant decay in rotor and engine RPM, triggering the low-RPM warning system. Recognizing that climbing safely was not possible due to surrounding topography and insufficient power margins, the pilot initiated a precautionary landing in an uneven agricultural field.

A combination of high density altitude, tailwind gusts shifting into a left crosswind, and the aircraft's nose-heavy configuration resulted in a loss of control during the flare phase. The helicopter struck the ground with its nose section and skid strut, rolling to the right before coming to rest.

Outcome

The aircraft sustained substantial structural damage, including separation of the tail boom and main rotor blade deformation. All four occupants evacuated without injury.
