What happened
On March 23, 2007, an Eurocopter AS350 B3 helicopter, registration HB-ZEJ, was performing commercial VFR operations near Cagetto, Brissago. The mission involved transporting materials between a loading site and a construction site located within a residential area.
During the ninth rotation of the day, the crew was tasked with relocating a palm tree weighing approximately 150 kg. The ground crew member used a sling method to secure the plant. As the pilot began to lift the load, the palm tree slipped from the sling and fell. The falling object struck a gardener who was present near the work area, resulting in one serious injury.
The investigation
Investigators examined the rigging techniques and ground safety protocols used during the operation. The investigation confirmed that the pilot was properly licensed and the aircraft was within its operational limits. The weather conditions at the time were favorable and did not contribute to the accident. /nTechnical analysis showed no mechanical failures in the helicopter or the lifting equipment. However, the investigation focused on the method used to secure the specific load and the monitoring of the danger zone by the ground crew.
