Investigation Report
The Dirección General de Aeronáutica Civil (DGAC) of Guatemala, through its Accident Investigation Unit, released the final report of the accident on 23 February 2026. The report (A-05-2025) covers the accident involving a Cessna 172RG aircraft with Guatemalan registration TG-ITA. The occurrence took place on 20 April 2025 on the runway of La Aurora International Airport, located in Zone 13 of Guatemala City.
Report Structure
The report follows the standard format outlined by ICAO Annex 13. It consists of 10 sections and an introduction. Section 1 provides factual information, including a synopsis of the flight, location of impact, injuries to persons, damage to aircraft, other damage, personal information, aircraft information, meteorological information, navigational aids, communications, aerodrome information, flight recorders, wreckage and impact information, medical and pathological information, fire, survival aspects, tests and research, organizational and management information, additional information, and useful investigation techniques. Section 2 analyzes general aspects such as flight operations, crew qualifications, operational procedures, meteorological conditions, air traffic control, communications, and navigational aids. Section 3 covers aircraft information, including maintenance, performance, weight and balance, and systems. Section 4 addresses flight recorders. Section 5 deals with human factors, both psychological and physiological. Section 6 discusses survival aspects, including rescue and firefighting service response, injury and casualty analysis, and survivor aspects. Section 7 presents conclusions, Section 8 lists probable causes, and Section 9 provides safety recommendations (RSO 01-A-05-2025 and RSO 02-A-05-2025). Section 10 contains annexes.
