What happened

On Saturday, 27 June 2026, a Piper PA-28-180 registered ZS-ESZ departed Lanseria International Airport (FALA) for a visual meteorological conditions (VMC) training flight. The aircraft, operated by Bird Aviation under a hire-and-fly agreement, was on an hour-building exercise intended to return to the departure aerodrome. After take-off from Runway 07 at 1325Z, the aircraft entered the Johannesburg General Flying Area. Eyewitnesses reported observing the aircraft flying unusually low and performing unfamiliar maneuvers before the engine silenced. Witnesses also heard sounds consistent with an attempted engine restart during descent. The aircraft subsequently flipped, spun to the left, and impacted the ground at a farm in Elandsfontein, south-east of Fochville. Two fatalities were confirmed; the pilot and passenger did not survive the impact, and the airframe was destroyed.

The investigation — what SACAA AIID examined and established

The South African Civil Aviation Authority (SACAA) Accident and Incident Investigations Division (AIID) classified the event as a Category 1 accident. Investigators examined the wreckage, which was found in a steep nose-down attitude with high-speed impact characteristics but limited forward momentum. The debris field was localized, with no significant ground scars other than the impact crater. Examination of the propeller revealed asymmetrical damage consistent with a non-rotating state at impact, corroborating witness accounts of engine failure. The flight controls were largely intact, though both ailerons were found in the down position and could not be moved. The undercarriage collapsed rearwards, and severe compressive damage to the wing leading edges ruptured the fuel tanks. No pre- or post-impact fire was detected. The pilot’s seat belt was not fastened at the time of impact, resulting in ejection from the cockpit.
