The investigation determined that the primary causes were an unstabilized approach with excessive vertical speed and airspeed, combined with a tailwind component exceeding operational limitations. Contributing factors included inadequate manual braking technique resulting in asymmetric brake application, poor Crew Resource Management due to the first officer’s failure to assertively monitor and call out deviations, and the captain’s reliance on manual skills over automation.
