- 2025-09 Air carrier technician reported that during training on a B-757; they noticed that two doors were still in the armed position. They disarmed the doors and the escape slide did not deploy.
- 2025-08 B757 Captain reported full lavatory waste tanks during cruise on an international flight with one lavatory inoperative for dispatch. The flight diverted to an alternate airport for lavatory servicing where maintenance determined the lavatories were not previously serviced prior to departure.
- 2025-06 B757-200 First Officer reported girt bar failed to retract when ground crew opened door. Escape slide pulled out of disarmed door but did not deploy.
