Accident Details
During the takeoff roll, the pilot-in-command was moving the flap control lever up and down in an attempt to effect a lift-off. The airplane never left the runway and continued beyond its end. It ran off the end of a ten-foot embankment and traveled approximately 140 feet before striking a bank. The impact caused the aircraft to nose over into a lake.
Pilot Statement
The pilot-in-command stated that no mechanical malfunction was discernible. The pilot's account indicates that the flap lever manipulation was part of the takeoff procedure, but the aircraft did not become airborne.
Wreckage Examination
Examination of the wreckage showed that the flaps were in the up position. Additionally, the throttle friction had come completely unscrewed from its shaft and was dangling on the throttle shaft. The status of the flaps and the throttle friction condition were observed during the post-accident inspection.
