Narrative
The pilot began the takeoff on runway 23 with a wind from 160 degrees at 13 knots. The tower controller observed the aircraft turn sharply to the left after lift-off and subsequently impact approximately 250 feet left of the runway. According to the controller, the pilot appeared to have 'jerked' the aircraft off the runway, and the aircraft climbed no higher than about 25 feet above ground level before the impact.
Pilot Statement
The pilot sustained serious facial injuries during the accident. In a later report, the pilot stated he had no recollection of the accident or of the events immediately preceding it.
Examination
A post-crash examination of the aircraft revealed no evidence of any preexisting mechanical malfunction or failure. Examination of the seat found that it was securely attached to the seat tracks and that the latch functioned properly.
