Incident Details

The incident occurred during the takeoff roll of a Boeing 767-232. An uncontained failure of the number 1 engine high pressure turbine wheel took place. As a result, portions of the turbine disk exited the engine case, causing a rupture that measured 4 inches by 16 inches. The disk and the second stage nozzle fragmented into pieces that became shrapnel-like projectiles. These projectiles pierced the engine core cowling and the surrounding engine pylon mount.

Response

The first officer, who was flying the airplane, aborted the takeoff. After aborting, the aircraft was returned to the departure gate.

Investigation
