A Grumman G-21 crashed during its third takeoff after two years of inactivity. Witnesses reported engine power until impact. Flaps were extended to 30 degrees, contrary to normal takeoff practice.

Background

According to the investigation, the Grumman G-21 involved in the accident had not been flown for two years. The pilot also had not flown this type of aircraft during that same period. Before departure, the pilot stated that three takeoffs and landings would be necessary to regain currency.

Flight Sequence

After takeoff, the pilot flew the airplane for approximately one hour and performed two full-stop landings. During the third takeoff, the airplane lifted off after a short ground roll and assumed a nose-high attitude below 100 feet. It then rolled left, struck the ground in a steep descent, and was consumed by fire.

Witnesses reported that the engines were producing power until the moment of impact. Post-crash examination of the engines and propellers revealed evidence of rotational damage, consistent with power at impact.

Investigation Findings

Examination of the flap actuators showed they were extended to a position corresponding to 30 degrees of flaps, or half flaps. Investigators interviewed four pilots experienced in the G-21. According to them, flaps are not normally used for takeoff in this airplane. They noted that the turboprop engines provide substantial power relative to the airplane's weight, especially when lightly loaded, and that the G-21 tends to become airborne quickly with flaps extended.

No preimpact mechanical problems were identified during the investigation.
