Background
During preflight checks, water was discovered in the aircraft's fuel tanks. Additional draining was performed until no further water appeared. The pilots then boarded and initiated a takeoff.
First Engine Failure
As the takeoff roll began, the engine sputtered, prompting an aborted takeoff. After the aircraft cleared the runway, the engine lost total power, and subsequent restart attempts were unsuccessful.
Operator Inspection
Ground personnel pushed the aircraft to the operator's hangar, where an inspection was conducted. Additional water was found in the tanks, which was drained, and the engine was run successfully. The operator stated the aircraft was "OK to fly," and the pilots boarded for departure.
Second Engine Failure and Forced Landing
Shortly after becoming airborne, at approximately 200 feet altitude, the engine lost power again. During the forced landing, the aircraft struck a power line and descended to the ground, where it nosed over.
Post-Incident Examination
When the aircraft was placed upright, additional water was found in the tanks. Examination of the fuel farm used for refueling revealed that the filter was collapsed and the fuel was contaminated with water.
Regulatory Findings
The Federal Aviation Administration (FAA) reported that the operator did not have a fuel management program in place nor the appropriate equipment to maintain a fuel farm.
Manufacturer's Guidance
According to the aircraft manufacturer, "If excessive sampling is required, the recommended procedure is to completely defuel, drain and clean the airplane fuel system, and attempt to discover where and how the contamination originated before the airplane flies again."
