Accident Overview
A test flight was initiated after the re-installation of an O-360-A4J engine following a major overhaul. The aircraft had been operated several times since the installation, with all adjustments completed. Prior to the flight, the fuel tanks were topped off.
Flight Sequence
The pilot-in-command occupied the right front seat, while a pilot who also held an airframe and powerplant (A&P) mechanic certificate and an inspection authorization (IA) occupied the left seat. The mechanic had performed the engine overhaul and signed off the annual inspection.
After a complete engine run-up and magneto check, the airplane taxied for takeoff on Runway 15. The takeoff was normal, with a rate of climb of 700 feet per minute at 70 knots indicated airspeed. As the airplane approached the departure end, the pilot initiated a 30-degree left bank while continuing to climb. The engine was running well until it suddenly stopped.
Loss of Power and Landing
The pilot in the right seat reported that the left-seat pilot did not react to the sudden loss of engine power. The right-seat pilot stated, "I've got it," leveled the wings, and attempted to keep the nose level, resulting in a "pancaked-in" landing.
Investigation Findings
An FAA inspector examined the accident site. Ground scars and crushing damage to the forward fuselage were consistent with impact in a pronounced nose-low attitude. Fuel was present onboard, and flight control continuity was established. The engine was found inverted, buried under the nose of the airplane, with the propeller still attached. Neither propeller blade exhibited leading edge damage or "S" bending.
Probable Cause
The owner of the airplane reported that the sudden loss of engine power was due to the failure of the engine driven fuel pump.
