This summary draws on records from the U.S. National Transportation Safety Board (NTSB) historical archive; 1 related events involving the same aircraft type or operator are linked below.
The pilot lost directional control during takeoff on an icy runway, causing the aircraft to drift left and strike a snow berm before nosing over.
What happened
During the takeoff roll, the pilot reported that the airplane began to drift to the left of the runway centerline. The pilot applied corrective steering inputs in an attempt to maintain the centerline; however, the aircraft continued to deviate laterally due to the icy surface conditions. The airplane eventually collided with a snow berm located off the side of the runway. Following this impact, the aircraft nosed over.
The investigation
The post-accident examination revealed that the aircraft sustained substantial damage as a result of the nose-over incident. The icy runway conditions were a significant factor in the loss of directional control during the critical takeoff phase.
Probable cause
The pilot's failure to maintain directional control during takeoff on an icy runway, which resulted in the aircraft drifting off the runway and colliding with a snow berm.
