What happened
On April 9, 2009, at 7:34 local time, a BAE Avro 146RJ, registration PK-BRD, crashed while performing its second approach to runway 15 at Wamena Airport in Papua, Indonesia. The aircraft, operated by PT. Aviastar Mandiri, was conducting a non-scheduled cargo flight originating from Sentani Airport. The impact resulted in 6 fatal injuries to the crewmembers on board, and the aircraft was destroyed.
The investigation
The National Transportation Safety Committee of Indonesia led the investigation into the accident. The NTSB provided an Accredited Representative to assist the process, acting as the representative for the country of manufacture and design for the aircraft's powerplants.
