Accident
On April 9, 2009, at 7:34 local time, an Avistar BAe-146 aircraft, registration PK-BRD, crashed during its second approach to runway 15 at Wamena Airport, Papua, Indonesia. The airplane was destroyed. All six crew members onboard were fatally injured.

Investigation
The investigation is being conducted by the National Transportation Safety Committee (NTSC) of Indonesia. The National Transportation Safety Board (NTSB) has assigned an Accredited Representative to assist the investigation under the provisions of ICAO Annex 13, as the Country of Manufacture and Design of the powerplants.
