Accident Details
On April 9, 2014, at 0025 universal coordinated time, a Quest Aircraft Company Kodiak-100 aircraft, registration PK-SDF, was performing a takeoff from Doyo Baru Airstrip in Jayapura, Papua, Indonesia. During the takeoff, the airplane lost altitude and subsequently struck the airport fence. After striking the fence, the aircraft came to rest on a roadway. The aircraft was operated by Adventist Aviation under the pertinent civil regulations of the Government of Indonesia.
Injuries and Damage
The accident resulted in fatal injuries to the pilot and one passenger. Two passengers sustained serious injuries. Additionally, three passengers and two people on the ground received minor injuries. The airplane itself was substantially damaged.
Investigation
The investigation into the accident is under the jurisdiction of the Government of Indonesia, specifically the National Transportation Safety Committee (NTSC).
