What happened
On February 26, 2016, a Piper PA-34-220T Seneca V, operated by Aircraft Management and Consulting sp. z o.o., was performing a flight from Herzogenrauch (EDQH) to Warsaw-Okęcie (EPWA). Following takeoff, the pilot attempted to retract the landing gear. After reducing the airspeed to 107 knots, the pilot made several attempts to retract the gear, but the mechanism failed to respond.
Due to the failure to retract the landing gear, the pilot decided to continue the flight to the destination airport with the gear in the extended position. To maintain safe operations, the airspeed was reduced to 128 KIAS. The aircraft subsequently completed the landing at Warsaw-Okęcie without further incident.
The investigation
The investigation was conducted by the operator. The inquiry focused on the sequence of events following the takeoff and the repeated unsuccessful attempts to actuate the landing gear mechanism during the climb and en route phases.
